The Program of All-Inclusive Care for the Elderly, an American health care program, provides comprehensive health services for the frail elderly. To receive care under PACE, seniors must be residents of nursing homes under the state Medicaid system. Eligibility for the PACE program is subject to several conditions. Lok

On Lok adds a social care center to its On Lok program. It provides elderly people with housing assistance, in-home care and home-delivered meals. These services go beyond the On Lok model and offer social support as well as basic medical care. On Lok's growth is supported by federal funding from the Department of Health and Human Services. It can now develop a national model of all-inclusive elderly care. On Lok supported the formation of the National PACE Association. There are now 21 PACE program operating in nine US states.


PACE Southeast Michigan

The PACE Southeast Michigan program offers all-inclusive care that provides a continuum to seniors who want to live in their own homes. These services include counseling, therapy, meals, and physical therapy. In addition, they provide prescriptions, outpatient services, and behavioral health and substance abuse care. PACE Southeast Michigan has many other services. They offer transportation, recreational activities and home maintenance.

PACE Programs are pre-paid capitated plans that are designed to cover all Medicare- and Medicaid-covered services under one plan. Providers must provide equal care to all Medicare eligible clients and Medicaid-eligible patients to be eligible for this program. Medicare and Medicaid reimbursements for PACE are based on prospective capitation. To be considered a provider of PACE, a facility must have good standing with federal and state insurance programs.

A recent study looked at the patterns of disenrollment in the PACE program that provides all-inclusive care to the aging population.
